Background Check Process Instructions

Follow these steps to complete the background check process as required by SKPD:
1. Access the Online Form by clicking on or navigating to: https://form.jotform.com/232796071647061
2. Ignore any notices indicating the form is for SK residents only. The police will process background checks for any address.
3. For the first question about fingerprints, select or check No.
4. For the second question, mark Yes. A drop-down form will appear for additional details.
5. In the form, when asked for the requesting agency, enter: UFD.
6. When asked who the results should be sent to, enter: Marylou Butler, and use the email: ufdoffice@unionfiredistrict.com.
7. Review your entries for accuracy, then submit the form as directed.

Due to the nature of Firefighting you will be placing yourself in situations that are uncommon to the general public. You will be trained and be required to use respirators in Escape, Emergency Rescue and situations determined by incident. You will be working under hot conditions (temperatures above 77 Degrees F). You will be working in humid conditions. You will be working in hazardous conditions such as confined spaces involving life threatening toxins and smoke.
